Substitution Forms

The online substitution form must be completed by your academic advisor. You must make an appointment with your academic advisor for the appropriate substitution(s).

Departmental Exit Exam

The departmental exit exam is administered each semester. You may register for the exam by clicking the button below and filling out & submitting the form before January 27. Specific details about time, location, and exam information are given after submission of completed registration form.

Graduation Cap and Gown Measurements

Perspective graduating students must be measured to ensure proper size for graduation cap and gowns. Obtain your measurements by visiting the James L. Bolden Campus Union Multipurpose Room.

Student Clearance Form

This form may be obtained from the Office of Student Affairs located on the second floor of the Student Union Building. University clearance begins April 17, 2017 .
